If you have given us your email address at the time of registration, you will be able to create a portal account and access your medical history.

In the portal you will be able to complete and update your medical history (24 hours prior to your upcoming appointment). This will allow you to have access to the summary of past appointments and view upcoming appointments dates and times.

Please update each tab along the top of the screen and be sure to hit the SUBMIT button after every page to ensure all of your information is saved.
